Students must meet these criteria to be eligible.  Please initial _______ if all the statements (1-5) below are true.
  1. * I am a resident of one of the states where the organization has participation at time of selection and a U.S. citizen (i.e., Mississippi, Alabama, or Georgia);
  2. * I have been accepted, enrolled and will be attending a college in the United States in the Fall;
  3. * I am of African-American ancestry;
  4. * I have a GPA of 2.5 or higher, with proof submitted with application (i.e. transcript).
  5. Choose One. * I am a college sophomore or graduating high school senior (attach proof of acceptance/enrollment).
